By Residents:                                                                                         At the end of the second week on the medical service, residents are responsible to complete a written two page reflective self-assessment and to share this with the teaching attending.  The teaching attending will add comments and return the self-assessment to the resident as mid-rotation confidential "formative" feedback:

Residents are also asked to complete anonymous evaluations of faculty, the training program, and evaluations of their peers:

Residents are encouraged to anonymously report to the program director violations of NY State Department of Health supervision and duty hour regulations:

When an error almost occurs (a "near miss"), it can be anonymously described so that changes to the system which permitted the "near miss" can be attempted:


By Faculty:                                                                                     Faculty provide confidential (1) "formative" feedback mid-rotation by responding to the resident self-reflection described above.  Faculty are also expected to assess resident performance by directly observing residents during the (2) "mini-CEX" and by performing (3) chart reviews.  Faculty should then verbally synthesize these global observations during end of rotation face-to-face "summative" feedback to the resident.   Faculty also provide written feedback to the residents when they complete the end of rotation global assessments that are sent by email via MyEvaluations.com.   "Voluntary" evaluations can be completed at anytime by logging on to MyEvaluations.com.   For quick notification to the program directory of praiseworthy behavior (or concerns), a "Praise or Warning" note can be quickly completed online:
